Medical Record Requests
Patients or authorized representatives may request copies of pre-hospital patient care reports from calls where Montgomery EMS responded.
Complete the Authorization for Release of Health Information form and submit by mail or email. Records are typically returned within two weeks.
Requesting your pre-hospital care report
To request the release of protected health information such as a pre-hospital care report, follow these three steps.
Complete the Authorization Form
Download and complete the Authorization for Release of Health Information form. All required fields must be filled in and the form must be signed by the patient or an authorized representative.

Send your completed and signed form using one of the following methods:

- • Mail: Send to the address below. Include a self-addressed stamped envelope if you wish to receive records by return mail.
Receive Your Records
Records will be returned via the method indicated on your authorization form. Please allow up to two weeks for processing. For status inquiries, contact records@mems47.org.

Records from MICU paramedic response
In some emergencies, Mobile Intensive Care Unit (MICU) paramedics also respond alongside our crew and provide advanced medical care. When paramedics are involved, they generate a separate pre-hospital care report. These records must be requested directly from the paramedic organization — not from Montgomery EMS.
Most paramedic responses in Montgomery Township are provided by one of the following:
Robert Wood Johnson University Hospital Somerset MICU
Contact RWJ Somerset directly to request paramedic care reports for responses involving their MICU unit.
Capital Health / Mercer LifeCom MICU
Contact Capital Health or Mercer LifeCom directly to request paramedic care reports for responses handled by their unit.
